1. Voluntary customer request

I am the adult purchaser or authorized legal owner of the identified MIA FOUR. I voluntarily request evaluation for an optional speed-limiter adjustment that may permit speeds up to approximately 55 MPH on an eligible configuration.

I understand that availability, technical capability and approval depend on my exact model, manufacturer specifications and any applicable restrictions. AMPED OUT MACHINES may refuse or delay this request.

2. Understanding of increased risk

I understand that increasing the permitted speed can materially affect handling, braking distance, stopping time, stability, tire loading and the severity of any collision.

At higher speed, a mistake, obstacle, equipment problem or terrain change may lead to loss of control, serious injury or death.

• I am responsible for choosing appropriate riding areas and speeds, following applicable laws and site rules, and complying with manufacturer instructions.
• I will use appropriate protective equipment, including a properly fitted helmet, and will not allow an unqualified or unauthorized person to operate the vehicle.
• I understand that a 55 MPH-capable configuration is not an assurance of safe operation at that speed and does not establish permission for public-road use.

3. Customer responsibility; assumption of risk

I knowingly accept the inherent and increased risks associated with my decision to request and operate the vehicle with a higher-speed setting.

I accept responsibility for my operation, supervision, maintenance, compliance with law and any resulting loss, damage or injury to the extent caused by my acts or omissions.

4. Warranty acknowledgment and limitations

I understand that increased-speed operation and any related modification may fall outside the manufacturer's specified operating conditions and may affect warranty coverage.

Damage, defects or failures caused by or attributable to the speed-limiter adjustment or higher-speed operation may be excluded from coverage under applicable written warranty terms and law.

I understand that AMPED OUT MACHINES does not promise warranty coverage for such unlock-related damage.

Any voluntary, discretionary exception offered by AMPED OUT MACHINES must receive express written approval from both MIA Dynamics and AMPED OUT MACHINES before it is treated as an approved exception.

Manufacturer obligations and any unrelated warranty rights remain subject to the applicable warranty and governing law.

5. No promise of performance or approval

I understand that the requested speed is not guaranteed. An adjustment, if technically supported and specifically authorized, may vary with model, battery level, terrain, load, tires, maintenance and conditions.

Submission of this form is a request only; it is not authorization, proof that a modification was performed or confirmation that a vehicle is approved for any particular use.

6. Authorization, documentation and record

I authorize AMPED OUT MACHINES to review this request and, only if separately approved, communicate with me about the proposed adjustment and any required inspection or documentation.

I consent to use of my electronic signature for this request and agree that the completed form and associated records may be retained as evidence of my acknowledgments.

I understand that I should keep a copy of the completed agreement.

7. Governing law; non-waivable rights

This agreement is intended to operate only to the extent permitted by applicable law. No term is intended to waive legal rights that cannot be waived, cancel an independent manufacturer's enforceable duties or exclude liability where prohibited by law.

If a clause is unenforceable, the remaining lawful clauses are intended to remain in effect.

8. Customer confirmation

By submitting the WPForms acknowledgment and my signature below, I confirm that I am at least 18 years old, am authorized to request this change, have read and understood the entire agreement, had an opportunity to ask questions and voluntarily request review of the increased-speed setting.
